The Case Manager provides the following support:
Intensive, Portable And Client-Directed Support
Linkage With Resources In The Community And Advocates Whenor Where Necessary
Assistance To The Client In Order To Define His/Her Needs And Goals
Collaboration With The Client To Achieve Goals
Collaboration with family as appropriate

The Case Manager has several functions:
Assertive outreach and client identification
Forming “therapeutic” relationships in the context of the service

Supportive and culturally/racially appropriate counseling
