Here click on the credit cards
You will be on credit cards page
Here click on apply now
Here click on the download form


You’ll get the list of form
Click on the one you want
A PDF will download






Take the print of the form
Fill the form
Submit it with the documents to nearest branch of BOB

Documents required:
Identity Proof : Copy of Passport/PAN Card/UID/Driving License/Voter's Identity Card
Income Proof : Latest Income Tax Return/Form -16/ Salary slip (Only in case of Govt. /PSU sector employees)
Address Proof : Copy of Passport/Electricity/Fixed Line Bill/UID/Bank passbook of PSU Bank
Colour Photograph
Copy of PAN Card
Last 3 months bank Statement
