As you have asked about the Chennai Institute of Mathematics application form, I want to tell you that
Applications for August, 2016 will open in early March, 2016.
Entrance examinations for all programmes will be held on Wednesday, 18 May, 2016.

When admissions are open, you can apply online or request a printed application form, either one by post or in person.

For all the programmes, candidates will have to take an entrance examination on Wednesday, 18 May 2016.

Ahmedabad, Allahabad, Bangalore, Bhubaneswar, Calicut, Chennai, Coimbatore, Delhi, Guwahati, Hyderabad, Imphal, Indore, Kolkata, Madurai, Mumbai, Nagpur, Patna, Pune, Ranchi, Shillong, Silchar, Srinagar and Trivandrum.

It offers following Bachelor courses:

B.Sc. (Hons.) in Mathematics and Computer Science (3 year integrated course).
B.Sc. (Hons.) in Mathematics and Physics (3 year integrated course).
