As you have asked about the Mortgage Banker’s Association (MBA) Evidence of Insurance form, I am giving you information about it, check below for the details

MBA Evidence of Insurance - Commercial Property Form is designed to provide details of property and casualty insurance coverage for commercial and multifamily properties.

The form is available for use by MBA member firms and members of the insurance industry providing property and casualty insurance coverage information in connection with the financing or servicing of commercial and multifamily properties by MBA member firms.

Since state requirements may dictate the filing and/or approval of evidence of insurance forms prior to use,

MBA has compiled the state-level requirements from a variety of third-party sources as of June 2011 and as a matter of general guidance.

Please also note that the MBA Evidence of Insurance - Commercial Property Form is intended to be used only in compliance with state requirements and is not intended (among other things) to extend, amend or alter the coverage, terms or conditions afforded by the policies.
