• Children applying to middle school will undergo Skill Assessment Program
• Record of vaccination / immunization
• Student should have attained minimum age requirement for the grade applied.
• Proof of residence of parent
• Original Transfer Certificate
• Medical Certificate
• Attested True Copy of the Birth Certificate
• 2 copies of passport size photographs and 3 copies of stamp size photographs
