The Pujab University is provides a membership application form. As you want here I’m giving you membership application form for gym:

University is provides instruction for members are, Members are required to deposit their identity cards with the attendant before entering the Gymnasium.

Eatables are not allowed to be taken inside the Gymnasium Hall.
