I want the Application Form For Issue Of Original Diploma Certificate of Utkal University so can you provide me?

Any student of DDCE of Utkal University who has passed any degree/ diploma examinations in 2009 or earlier ( BA/B.com/MA/ IMBA/ BBA/ PG DIPLOMA/ MBA/B.Sc ITM/MCA) etc and has not obtained original degree may apply for obtaining the same.

Amount of fees deposited :

Rs.150/- ( Rupees. One hundred Fifty ) only through Demand Draft Drawn in favour of the Comptroller of Finance, Utkal University, payable at Bhubaneswar.

The students are required to submit the application form at DDCE along with the following documents.

Filled-up original application form.
Photocopy of mark sheets of all semesters/years.
Photocopy of provisional certificate.
Photocopy of Utkal University registration Number.

Those who have passed any examination before 2002 are required to deposit Rs 670 /- through bank challan at State Bank of India, Utkal University, in favor of Comptroller of Finance for the purpose of original degree.
