Documents needed with the application form:
(a) BDS & MDS degrees.
(b) Matriculation Certificate for proof of Date of birth.
(c) BDS final year mark-sheet.
(d) Internship completion certificate.
(e) Dental Registration Certificate duly renewed upto 31 Dec 2013.
(f) Two additional identical passport size recent photographs.
(g) Authority under which BDS/MDS Dental College is recognized by DCI.
(h) ‘Number of Attempts in final year BDS’ Certificate from head of institute.
(j) Any other relevant documents.
ELIGIBILITY:
Candidates must have passed BDS/MDS (with minimum 55% marks in final year
BDS) from a College/University recognized by Dental Council of India
(DCI).

Candidates must have completed one year rotatory Internship recognized by DCI

Candidates must have been in possession of Permanent Dental Registration Certificate
Candidates must have been less than 45 years of age

Candidates must be medically fit in all respects for service in the Armed Forces.
