Registration Certificate for Nursing, Midwifery / Alternate course and additional Nursing qualification if any.

Experience certificates after B.Sc. (N) / B.Sc. (N) for Trained Nurses

Transcript of B.Sc. (N) / B.Sc. (N) for Trained Nurses

Mark lists of B.Sc. (N) / B.Sc. (N) for Trained Nurses

Certificate indicating date of birth

General educational qualification certificates (10th and +2)

Current medical fitness certificate

No objection certificate from the latest employer

Merit certificates.

Sponsorship obligation completion certificate (Where applicable)

B.Sc Nursing Course:

The completed forms to get the admission to the institute should reach the Principal. C.S.I. Jeyaraj Annapackiam College of Nursing,•
