One can use Fitness to Practise Declarations form by GPHC (General Pharmaceutical Council) if one is a pharmacist or pharmacy technician registered with it to tell about any issues which may affect one’s fitness to practise, including: Cautions or convictions Investigations or fitness to practise proceedings with other regulatory bodies Issues connected with academic or training career. One does not need to declare fixed penalty notices for offences such as speeding. Use the form for ad-hoc declarations. Do not use it as part of annual renewal. One should renew registration using myGPhC. If one has already declared a matter as part of application to the register or annual renewal, one does not need to declare it again unless something has changed.
