So I will not be able to give the admission forms but I will give you other details of admission in B.Tech in Jaypee University of Information Technology:

Admission Process in 4 Steps:
Step‐1. Register/Apply for Jaypee Education System – Fill online or hard copy of application (OMR).
Step‐2. Announcement of Merit List based on overall all India rank in JEE (Main) 2015 (for all).
Step‐3. Counseling shall be held for all from 16‐20 July depending upon JEE (Main) results. Monitor and view the websites for details without fail.
Step‐4. Admission and commencement of classes by 27 July, 2015.

ELIGIBILITY CRITERIA:
Candidates must have passed class 12 or its equivalent (as defined by JEE) with Physics
and Mathematics as compulsory subjects and any one of Chemistry, Biology, Biotechnology, and Computer Science as optional subjects.

Criteria of Qualifying Examination:
All candidates who are offered provisional admission will be required to produce proof of having passed the qualifying examination (10+2 or equivalent) to the University authority latest by September 30, 2015 failing which, such provisional admission will stand cancelled.

Application Form Process:
Candidates can get the application form and brochure from any of the three campuses by cash payment of Rs 1000/‐or sending a bank draft of Rs. 1000/‐ to JIIT favoring Jaypee Admissions Account, payable at Noida/Delhi.

Candidate scan also get the application form and brochure from selected bank branches of IDBI and PNB as per he details loaded on the institutional web sites.

(b) Apply on line—Rs 900/‐ (Discount of Rs 100)

Important Dates:
(a) Last date of receipt of applications ‐ up to 30 June 2015
(b) Merit Lists to be announced ‐ within four days of declaration of JEE (Main) results by the CBSE.
(c) Issue of Call letter thru E mail ‐ within 1 day of merit lists being displayed
(d) Counseling (Common to All Institute) ‐ dates will be indicated above along with the merit lists (tentatively JIIT Noida 16‐17 July & JUIT, 19‐20 July).
(f) Registration and Commencement of classes ‐ Shall be mentioned in admission letter. i.e. 27 July.

The dates which are mentioned above are the dates of 2015 and these dates can be taken as tentative for the year of 2016 as the dates for the year of 2016 have not been announced yet
