Go through all instruction about MP PMT application form

Fill the registration form carefully without any mistakes.

Enter your personal details like Candidate full name, address, phone number, Email
address and educational details correctly.

Scan your Photo, signature.

If any error in your application forms so your application form will be rejected by
Vyapam authority.

Pay fee via net banking, credit card or debit card.

Click the button of “SUBMIT”.

Take the printout of application form and receipt of payment for further use.

Eligibility

You must be domicile resident of Madhya Pradesh.

You should be completed 17 year on or before 31 December of the year of examination.

You mus t be 12th passed form the Bio-stream

Document for Vyapam MP PMT Counseling

Category certificate (SC/ST/OBC/Other) signed by the authorized authority.

Character certificate

Certificates of 10th

Certificates of 12th class.

MP PMT 2016 Admit card.

2 passport size photographs color and black & white.

Other supportive documents mentioned in MP PMT 2016 counselling form

MP PMT 2016 Rank Card / Score card (if applicable).
