Steps to apply for birth certificate:

Visit nearest meeseva centre and then need to request them for new birth application form.
Fill the form with
Your name,
Wife name,
Baby date of birth,
Hospital details
Birth place,
Delivery date like that details.

Attach the application with Hospital Delivery confirmation receipt.
Now submit the application form to the meeseva operator.
From the date of applying in below 15 days birth certificate will be issued with out name.
After 3 months again visit meeseva centre and again apply for birth certificate with name.
In this time attach the application with old birth certificate xerox copy.
