To get the Provisional Certificate of Kuvempu University, you have to fill the prescribed application form and then send it to the University. Here I am providing you the form. Provisional Certificate application form   Separate applications must be used for each year. (i.e. 1st year, 2nd year and 3rd year etc. Incomplete applications are liable to be rejected. Attach the following with the application form- D.D. for the prescribed fee Attested Xerox copies of Marks Cards of all the years (Including Entry Level Exam Marks Card Xerox Copies) P.P.C is issued only up to the date of Convocation from the date of announcement of results. After the ensuing Convocation, the Degree Certificate will only be issued.
