Would you please provide me Merit-Cum- Means scholarship form issued by Indian Institute of Technology (BHU) and also tell me eligibility criteria and instructions of scholarship?

Eligibility
a) Merit - As per rank obtained in JEE AIR/PMT Screening Marks – 2013.

b) Means – The annual income of a student’s Parents/Guardians and his/her family from all sources in the financial year 2012-13 should not be more than Rs. 6.00 Lakhs.

Instructions
1. All the details must be filled up and no column be left unfilled. In case there is no Income against a particular head, the word ‘NIL’ be specifically written.

2. Total Income of the family should include only of the father, mother and the concerned student and no other members though they may be earning.

3. If father is dead, then mother’s income will be shown alongwith the income, if any, in the student’s own name from any source other than scholarship and stipend.
