Southwest Community Health Center is a safety-net-clinic providing basic health care to low-income uninsured individuals. Southwest Community Health Center is recognized by The Joint Commission as a Primary Care Medical Home Certified Organization. We are also a Federally Qualified Health Center and FTCA Deemed Facility. Mental Health Services- Mental health services offered include psychiatric evaluation, treatment and therapy for adults and children. Highly qualified clinicians offer individual and family counseling, group therapy and anger management. A wide array of social services, including patient advocacy, entitlement program enrollment and case management are offered.
