Affix Recent Passport Size Photograph attested by the ‘Head of the Institution’

All entries should be made in English block letters in the Applicant’s handwriting.

Write one letter in each box and leave one blank space (gap of a box) between two items.

This form is to be submitted duly filled in, by the student, to the office of the Head of the Institution along with the following documents and fees to be forwarded to the University within three months of admission, or else a late fine @ Rs. 100/- (Rupees One Hundred only) per month will be charged :

Self attested Copy / Copies of pass mark sheet(s) of the previous qualifying examination(s)

“Migration Certificate” / “School leaving certificate” in original, as applicable.

Self attested Copy / Copies of certificate(s) in respect of SC/ ST/ OBC/ Physically Challenged status, where applicable.

Age proof certificate.

Letter of permission from the employer, if employed.

Registration Fee of Rs. 2,000/- (Rupees Two Thousand) only by a Demand Draft in favour of the ‘West Bengal University of Health Sciences’ payable in Kolkata
