Would you provide me Canara Bank education loan form?

About Educational loan:
Loan Amount:
Upto Rs. 10.00 lakhs for Studies in India
Upto Rs.20 lakhs for Studies Abroad
Interest Rate: 10.25%-12.5%
Pre Payment Charges 2% on the outstanding liability in case of take over of the loan by other bank / FI

Eligibility Conditions:
Institutions offering courses should have approval of Central Govt. & State Govt. /University affiliation/Autonomous Institutes/AICTE/DOEACC.

Repayment Conditions:
To be repaid in 5 to 7 years.
After commencement of repayment, the accrued interest is added to the principal amount and EMIs fixed on total outstanding amount.
